What does breathing have to do with neck pain?
Your neck contains accessory breathing muscles — the scalenes and the sternocleidomastoid — that are meant to help only during heavy exertion. When breathing becomes shallow and chest-led, which happens under stress and when sitting hunched, those muscles get recruited for every breath instead. At roughly 20,000 breaths a day, that is an enormous amount of unnecessary work for muscles already busy holding your head up.1
How do I check how I am breathing?
One hand on your chest, one on your belly. Breathe normally for a minute without trying to change anything, and notice which hand moves.
If the chest hand moves more than the belly hand, you are chest-breathing. If the belly hand rises first and further, the diaphragm is doing its job. Many people are surprised by the result, particularly at the end of a working day.
What is diaphragmatic breathing?
Breathing led by the diaphragm — the dome-shaped muscle under the lungs — rather than by lifting the ribcage with the shoulders and neck. When the diaphragm contracts and flattens, it draws air in and the belly moves outward. The neck stays out of it entirely.1
To practise: lie on your back with knees bent, one hand on your belly. Breathe in through the nose so the belly hand rises. Breathe out slowly through slightly pursed lips and let it fall. Five minutes.
Why does the exhale matter more than the inhale?
Because a longer exhale shifts autonomic balance toward the parasympathetic state, which lowers heart rate and baseline muscle tone. That is the mechanism by which slow breathing reduces neck tension: it is not stretching anything, it is turning down the signal telling the muscle to contract.2
Practically, make the exhale longer than the inhale. That single rule is most of the technique.

When should I use these?
The physiological sigh — two nasal inhales, the second shorter, then a long exhale through the mouth — is the one to use in the moment, because it takes about five seconds and works standing in a meeting.
The longer patterns are better as a scheduled practice: five minutes in the morning, five before bed, and whenever you notice your shoulders have crept up.3
Does this actually reduce neck pain?
It reduces the tension component, which for people whose neck pain is stress-linked is a large share of it. It does nothing for pain caused by a screen that is too low or by a nerve being irritated.
The useful question is whether your neck pain varies with your stress levels. If it does, breathing work will do more than another stretching routine. If it does not, look elsewhere first.
What if breathing exercises make me feel dizzy?
Slow down and breathe less deeply. Dizziness usually means you are over-breathing — taking in more air than you need — which is easy to do when consciously trying to breathe well.
The target is quiet, unforced, slightly slower than normal. If you are working at it, it is too much. Stop if dizziness persists, and mention it to a clinician if it happens easily.
